Lions Family Diabetes Camp

Join us for an exciting weekend at Triple R Ranch in Chesapeake, Virginia, from April 24, 25, & 26, 2026. This one-of-a-kind retreat is designed for individuals with diabetes, ages 6 to 18, and their families—including parents, siblings, and grandparents. The cost is $50 per participant attending. At least one parent, guardian or caregiver over the age of 18 is required to attend.

General Instructions for Completing the Registration Form

1. Complete all required fields. Fields marked with an asterisk (*) are mandatory. Be sure to fill them in before submitting the form.
2. Provide detailed and accurate information. This helps us tailor the program to meet your needs and ensure a safe and enjoyable experience. We will request both general and diabetes-specific medical information. To save time, we recommend having this information readily available for reference.
3. Use the “Additional Information” sections as needed. If a question requires further explanation, feel free to use the designated sections to provide more details.
4. Review your answers before submitting. Double-check your contact details, dates, and other important information to avoid delays or miscommunication.
5. Save a copy of your submission or use a separate document. Some browsers allow you to save or print a copy of your completed form for your records. Additionally, you may wish to answer some of the form questions in a separate document and paste the answers into the corresponding fields. This ensures your work isn’t lost if you need to pause and return to the form later.
